Abstract
Unplanned return after emergency-department (ED) discharge is an important outcome of acute renal colic, but reported rates depend on the follow-up window, the denominator, and whether returns to other hospitals are observable. Rates, reasons, and predictors of such revisits in adults were described. PubMed/MEDLINE, Cochrane CENTRAL, CDSR and Epistemonikos were searched on 2026-08-11 without language or date restriction; Embase, Scopus and Web of Science were unavailable. Adults aged 16 years or older discharged at the index visit, with the denominator restricted to discharged patients, were eligible. Two reviewers screened, extracted and appraised independently, a third adjudicated four. Appraisal used the JBI prevalence checklist and QUIPS. Synthesis was narrative, grouped by window then cross-hospital capture, with exact Clopper–Pearson intervals and no pooling. Of 333 records, 307 were screened after deduplication; 92 reports were sought, 28 not retrieved, 64 assessed, and 11 studies included. Eight study-window rows supported exact proportions: 18.6% and 22.5% at 7 days, 21.4% at 14 days, 10.6% to 14.7% at 30 days, 27.7% at 60 days, and 7.3% at 90 days. One further 90-day row was definition-limited, and four studies were non-estimable. Only one study reported counted reasons, pain accounting for 148 of 164 returns. No study was low risk overall on QUIPS. Reported proportions are not exchangeable across windows or capture systems, and no single overall rate is defensible. Studies should report an explicit discharge denominator, a stated window, a definition of unplanned return, and whether cross-hospital events were observable. Twenty-eight unretrieved reports limit sensitivity.

Key words: Renal colic, ureteral calculi, emergency service, hospital, patient readmission, systematic review, prognosis
